Job description

The position is responsible to assist the patient, their families and provider across all aspects of their health care continuum. The Care Services Coordinator is the liaison between the patient and medical provider and/or psychiatric consultant, with an optimum goal of meeting the patient's individualized needs, ultimately improving quality of life which will result in positive patient outcomes through communication, education and coordination. Is responsible for supporting and coordinating the mental and physical health care of with the patient's medical provider and, when appropriate, other mental health providers Essential Duties: - Support the department with Care Services Coordination for patients under direction of medical providers and administrative functions under Department Management. - Support the mental and physical health care of patients by closely coordinate care with the patient’s medical provider and/or other mental health providers. - Screen and assess patients for common mental health and substance abuse disorders using Mira Health Care Management software program. - Provide brief behavioral interventions using evidence-based techniques such as behavioral activ...
