Key Responsibilities: - Calls on physicians, hospitals, skilled nursing facilities, and rehabilitation discharge planners and management within an assigned territory to promote designated service lines. - Conducts pre-discharge visits to hospitals or other facilities at the request of physicians, and upon receiving physician orders, to make a preliminary determination of the need/eligibility for home care. - Maintains a detailed territory list, prioritizing accounts in accordance with the market/territory sales plan. - Maintains records in detailed territory management systems. - Provides leadership and accountability by coaching, developing, and retaining a high-performing sales team. - Demonstrates consistency in achieving both admission and cens...
