JOB TITLE: Case Manager: EMPLOYMENT TYPE: Per-Diem: SCHEDULED HOURS: 2nd shift flexibility: PROGRAM/LOCATION: Strickland; Manchester, CT: ABOUT THE PROGRAM: Strickland: An intensive residential program, providing 24-hour support services. Target population are individuals discharging from CVH. ABOUT THE POSITION: Case Manager: QUALIFICATIONS: Education: Bachelor's Degree Required Experience: Requires previous experience relating to and working with substance abusers. Previous work history must demonstrate responsible work habits. Licensure/Certification/Registration: Must have a valid state driver’s license. (Requires good driving record.) CPR/First-Aid Certification within six months of hire. - Maintains residents’ records and clerical records as assigned and in accordance with the Department of Mental Health and Addiction Services (DMHAS). - Provides individual case planning sessions.
