Whether administering medications, monitoring patient health, or mentoring Nursing Assistants, you’ll play a key role in enhancing the dignity, safety, and well-being of our residents. As a LPN, you will: - Monitor Resident Health and Document Changes – Obtain necessary information to initiate and update the Plan of Care for each resident. Observe and document changes in resident’s physical, mental, or emotional status. Communicate resident changes with the interdisciplinary team and document all observations. - Provide Direct Client Care – Administer medications as prescribed or other physician-ordered treatments.
