Job description

Overview The Coordinator, Adult Community Services position supervises a variety of community-based programs, including both single apartments and congregate housing with case management support, and homeless outreach. These service options help AltaPointe’s adult, outpatient consumers move forward in their recovery, access local resources to meet their unique needs, achieve a better quality of life and function as independently as possible in their community. Responsibilities: Supervises Adult Community Services program operations: (Arbor Court, VOA, Supportive Housing, Permanent Housing, Shelter + Care, and PATH): - Directs and organizes program activities - Establishes program goals and objectives that support the organization’s strategic plan - Identifies problems, evaluates alternatives, and implements effective solutions - Maintains and monitors electronic tracking of referrals - Manages wait lists - Evaluates referrals for eligibility, coordinating and staffing with members of candidate’s treatment team(s) - Staffs all referrals with supervisor, prior to program entry - Coordinates with Adult Community Services’ Property Manager regarding moves in and out of housing, furnitu...
