Description What We're Looking For: Are you a proactive problem-solver who takes pride in delivering meaningful work that makes a lasting impact? We're looking for a driven and detail-oriented professional to join our team as a Linkage to Care Navigator. In this role, you'll play a vital part in coordinating medical, prevention and other support services for people living with Hepatitis C and/or HIV in order to assure access to and retention in care. The ideal candidate values continuous learning, leads with a welcoming spirit, takes ownership of their work, and is passionate about supporting people and building stronger communities. Essential Job Duties: - Conduct case investigations and follow-up on reportable infectious diseases (HIV, HCV, STIs) in accordance with state and local guidelines. - Perform community outreach to identify, notify, and engage individuals at risk or diagnosed, including completing interviews and risk assessments. - Link individuals to medical care, treatment, and supportive services, and support retention in care through ongoing follow-up. - Coordinate with healthcare providers, public health agencies, and community partners to ensure continuity of care...
