Job Description Adheres to the personalized plan of care and assists in the delivery of personal care and activities of daily living, including assistance with ambulation, positioning and transferring that may require the use of a mechanical/ Hoyer lift. Assisting or providing total personal care related to toileting, bathing and perineal care, eating dressing and personal hygiene where indicated. Facilitate or support with Residents’ move in and transfers where applicable. observe, report and document on Residents care and services provided utilizing appropriate application systems or applicable paper tools. Observe and report changes in Residents/ condition/behaviors to the nursing supervisor; to ensure appropriate follow up and assessment. Immediately communicate medical emergencies to the nursing supervisor or contact emergency medical services as appropriate or as directed. Demonstrate an understanding of Resident abilities/ required supports while fostering and encouraging independence where Residents are capable to maximize self-sufficiency.
