Asian Health Services, founded in 1974, provides medical, dental, and behavioral health services to more than 50,000 patients. Our approach to well-being focuses on “whole patient health” for all. Job Summary: Care Neighborhood is an innovative program in collaboration with the Community Health Center Network (CHCN) to support and empower identified high risk-high needs patients to improve their health outcomes. The multidisciplinary team (Nursing staff, MD, LCSW, and CN CHW) works together to support patients and address the social determinants that impact health. These include food, housing, transport, and financial resources.
