As a Case Manager, you will oversee and coordinate all aspects of patient care, develop individualized care plans, and collaborating with an interdisciplinary team to ensure the highest standards of health and wellness. Responsibilities: Patient Assessment & Care Planning: Conduct initial and ongoing assessments of patients and their families to determine home care needs. Develop and update individualized care plans based on health assessments, including therapeutic, preventive, and rehabilitative actions. Direct Patient Care: Administer medications and treatments as prescribed, provide skilled nursing care per the State Nurse Practice Act, and implement preventive and rehabilitative procedures.
