Job DetailsJob Location: Morrison County Government Center - Little Falls, MN 56345Position Type: Part TimeSalary Range: $31.88 - $45.37 HourlyJob Category: Health & Human Services DepartmentPOSITION PURPOSE: The primary purpose of this position is to provide MnCHOICE assessments as part of the eligibility determination process for adults and children to access Home and Community Based Services (CADI/CAC/BI/DD Waivers, Rule 185 Case Management, PCA Services) and/or to determine needs and make referrals for other support services as determined by this assessment process. Duties include, but are not limited to, determining eligibility, assessment of strengths and needs, helping individuals in person-centered planning, arranging, coordinating and monitoring services all in accordance with applicable Rule, Statute, any other federal or state regulations and Agency policy. This is a part time, non-benefited position where the work demand can fluctuate based on the New Assessments and Reassessments case load each month.
