Job Description: - Permanent / Part Time position 0.4EFT - Hybrid work environment - Friendly and supportive team About the Role: An opportunity currently exists for an experienced clinician (Registered Nurse or Allied Health Professional) to join the Health Monitor Team on a permanent part time basis (0.4 FTE). The Health Monitor clinician will provide phoned based assessment, monitoring, short term care coordination and onward referral of clients that are recently discharged from hospital. Management strategies will be targeted to prevent avoidable hospital presentations. As an experienced clinician, the Health Monitor will demonstrate knowledge in the management of clients with a diverse range of issues both medical and psychosocial needs and be experienced in the coordination of care post discharge from the hospital setting. This role will work with the team to deliver high quality phone-based care to clients that have been recently discharged from acute services. This position will require weekend work.
