The Complex Care Kidney Transplant Social Worker plays a vital role in supporting patients and families through the kidney and pancreas transplant journey. In this role, you’ll provide specialized psychosocial assessment, care coordination, and education within a fast‑paced, highly collaborative transplant environment. You’ll work closely with transplant coordinators, financial teams, and a multidisciplinary care team to help patients prepare for transplant, navigate insurance and logistical challenges, and remain engaged throughout evaluation, waitlist, and post‑transplant phases. This position is well‑suited for social workers who enjoy complex medical work, value teamwork, and are comfortable navigating sensitive conversations with empathy and professionalism. Key aspects of the role include recognizing psychosocial patterns, identifying potential barriers early, and intervening proactively to support transplant readiness and continuity of care.
