The Case Manager will provide comprehensive wrap-around case management services to pregnant women, single parents, and families, with a focus on promoting self-sufficiency and healthy family outcomes. This position requires strong communication, organizational, and interpersonal skills to effectively support clients in accessing a range of services and resources. PRINCIPAL DUTIES: Conduct intake assessments and develop individualized service plans in collaboration with clients to address their needs and goals.
