Job DetailsJob Location: Prospect House - Bridgeport, CT 06604SUMMARY: In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Coordinator the Case Manager will assist in the provision of quality care for persons served by performing the following duties: HOURS: Monday - Wednesday 8:00 am - 4:00 pm; Saturday 8:00 am - 4:00 pm & Sunday 4:00 pm - 12:00 am. SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA). QUALIFICATIONS: - Knowledge of best practices in providing services for individuals with mental health and substance use disorders such as client centered care, motivational interview and trauma informed care. - Maintain boundaries and ethical standards understand and demonstrate an ability to maintain appropriate professional boundaries when working with client and uphold ethical standards, e.g, no dual relationships, minimal disclosure, etc. - Develop and document client centered care plans, record and monitor clients progress regarding HIV/AIDS, finances, employment, food, basic needs, housi...
