This position is eligible for a [prorated] $1,000 sign-on bonus. Job Summary: The Providence Center Residential Case Manager will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible. Schedule : Saturday first shift, 7:00am - 3:00pm, Sunday second shift 3:00pm - 11:00pm Duties and Responsibilities: Initiate treatment and advocate for consumer within network of care. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. Provide case management and support to consumers.
