Position Summary The Clinical Nurse provides safe, patient-centered nursing care in an ambulatory setting. This role performs routine assessments, supports clinical procedures, assists with care coordination, and reinforces patient education while contributing to a collaborative, team-based care environment . Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned. Clinical Assessment & Planning: - Perform holistic nursing assessments that consider physical, psychosocial, cultural, and environmental factors. - Identify nursing problems, establish measurable patient goals, and develop individualized plans of care. - Collaborate with physicians and interdisciplinary team members to implement and revise care plans as needed. - Evaluate and document patient progress, modifying interventions to support optimal outcomes.
