Description Hospice de la Luz is currently seeking a full-time Licensed Medical Social Worker to join our team to serve our patients currently enrolled in our hospice services in the Albuquerque, NM region and surrounding areas. The successful candidate will provide coordinated care to patients of all age groups, performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient, client, and family. This position offers an opportunity to make a significant impact on the lives of patients and their families in a supportive and dynamic environment.
