Housing Services Coordinator

Job description

Overview The Housing Services Coordinator is responsible for reviewing, approving, and supporting requests for housing-related services for Community Supports services offered by the health plan. This role ensures that housing related services are appropriate, align with program criteria, and support members stability and health outcomes. The position works closely with departmental clinical teams, care management teams, external housing providers, counties and community-based organizations to support access to safe, sustainable housing services and supports for members. Responsibilities: - Reviews and processes referrals for housing-related services (e.g. Housing Transition Navigation Services, Housing Deposits, Housing Tenancy and Sustaining Services, Transitional Rent, etc.) in accordance with DHCS guidance and plan policies. - Evaluates documentation (e.g. housing plans, household goods, tenancy barriers, etc.) for completeness, eligibility, and compliance. - Makes and ensures timely authorizations or determinations on service requests within plan policies. - Collaborate with clinical and non-clinical staff to educate, support, and assist with members with housing related needs...
