Description JOB SUMMARY: Within the context of a client centred care model and in accordance with established vision and values of the organization, works as a member of an interdisciplinary community health team, the Wellness Support Worker provides a variety of day-to-day physical, emotional and social supports, advocacy, life skills assistance, information, resources and demonstrations to designated population groups in the community. Reports back to interdisciplinary team on clients’ progress and condition. Establishes working relationships with various agencies. Co-facilitates support groups, cultural activities and public education. QUALIFICATIONS: Education & Experience Completion of a certificate in a Community Social Service or related program and two years of recent, related experience working with designated population group or an equivalent combination of education, training and experience.
