Family Support Coordinator

Job description

Overview The Family Support Coordinator creates, expands, and coordinates innovative community based services for individuals with developmental disabilities. Partners with individuals and families to develop individualized plans that will help them to meet their goals and lead full lives as active members of their communities. For this 37.5 hour position we offer Tuition Assistance, paid leave and sick, health Insurance , and a 20% Discount on Childcare Services for children and grandchildren of staff. Salary is $20-$22.56 per hour depending on experience. Responsibilities: - Ensure families receive timely, quality services - Serve as liaison between individuals/families, community support professionals, and Care Mangers as needed - Communicate regularly with individuals receiving services and/or families regarding services provided, - Support the development of Circles of Support - Attend individual’s Life Plan and COS meetings, as needed and outlined by OPWDD guidance documentation, to support each individual with managing their services. - Develop Staff Action Plans with individuals/families, Care Managers and community support professionals as needed, ensuring that Plans are w...
