Full Time (80 Hours Biweekly) Day Shift: Monday-Friday 8am-4:30pm Every 4th Weekend and 2 holiday a year General Summary: Provides professional nursing services to individuals in the home through a process of nursing assessment, planning, care, and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures and the Nurse Practice Act. Carries primary care management functions for assigned patients. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. May require specialized skills depending on VNA assignment. Duties and Responsibilities: Essential Functions: - Independently recognizes subtle changes in condition and modifies plan of care accordingly.
