Job Title : Care Navigator Department : Inpatient Psychiatric: Location : Clifton Springs Hospital - 2 Coulter Rd, Clifton Springs, NY 14432 Hours Per Week : 36 (12 hour shifts) Schedule : Days 0700-1930, Nights 1900-0730, Every other weekend rotation SUMMARY : Assist Unit Leadership in assessing patient’s functioning as well as developing and implementing treatment plans and services as patient’s needs warrant. RESPONSIBILITIES: Provides direct care services to patient on an individual and group basis, to include but not limited to activities of daily living, social, and psychiatric rehabilitation per treatment plan, as directed by Unit Leadership. Participates in the development of a comprehensive treatment plan sharing patient’s needs and wants as communicated by patient directly through patient surveys, collaterals, and treatment team. Documents and communicates patient response to treatment effectively and accurately and in a timely manner according to Program Policies and Procedures.
