Social Worker - Discharge to Assess-Acute Hospital Teams (Queen Elizabeth Hospital and Heartlands Hospital) Permanent: Grade C - £35,412 - £44,075 Golden Hello and retention payment - £1,000 + £5,000 per annum; see below for further details and conditions. Consultation grade - subject to formal evaluation under the Pay Equity Review Working 36.5 hours per week: An exciting opportunity has arisen to join the Acute Hospital Teams based at Queen Elizabeth and Heartlands Hospitals. The successful candidate will work across busy multi-disciplinary teams, supporting high-quality social work practice and, where required, contribute to other areas within the Discharge to Assess (D2A) service. A key element of this role is to strengthen collaborative working, promoting effective multi-disciplinary team engagement within a fast-paced environment. As part of the Discharge to Assess Social Work Teams, you will deliver person-centred practice, enabling individuals to maintain choice, control, independence, and resilience.
