Registered Nurse - Palliative Carelink 1 FTE: Thunder Bay Site One of a network provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking 1 FTE permanent Registered Nurse - Palliative Carelink. As a member of NorWest Community Health Centres, the Community-Based Palliative Carelink program, the Registered Nurse plays a vital role in delivering holistic, compassionate, and equitable care to clients with palliative needs across the community. Guided by NorWest Community Health Centres mission to work with the community for the community, this role prioritizes responsive, client-centered care and seamless coordination across multiple care settings including homes, shelters, outreach programs, and health system partners. This role serves as a central contact for palliative carelink clients, caregivers, and health care providers, supporting individuals throughout their illness trajectory with an emphasis on dignity, respect, and cultural sensitivity.
