Description POSITION SUMMARY: The Center of Excellence (COE) Case Manager is responsible for providing comprehensive case management services to individuals with a history of addiction and co-occurring disorders, particularly individuals with opioid use disorders (OUD), and providing specialized support to The Wright Center clinical staff. Responsibilities include a range of client-centered services that link clients with medically assisted therapy (MAT), primary care, psychosocial, and other services. Behavioral health case management ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the clients’ and other key family members’ needs and personal support systems. This position includes client-specific advocacy and/or review of utilization of services, including warm handoffs from inpatient, outpatient, and emergency. This position requires flexible working hours including evenings, weekends and holidays. REPORTING RELATIONSHIPS: This position reports to the Lead COE Case Manager.
