Job description
The Licensed Psychiatric Mental Health Nurse Practitioner will work alongside the integrated mental health care team and primary care team providing psychiatric medication management and support. Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. - Perform comprehensive psychiatric assessments, including history, mental status exam, and risk assessment - Diagnose mental health and substance use disorders in accordance with DSM-5 criteria - Initiate, prescribe, and manage psychotropic medications - Adjust treatment based on clinical response, side effects, and patient presentation - Monitor medication, adherence and effectiveness - Order and interpret laboratory tests as clinically indicated (e.g., metabolic monitoring, lithium levels) - Provide patient education on medication risks, benefits, and alternatives - Conduct follow-up visits focused on symptom management and medication response - Utilize standardized screening tools (e.g., PHQ-9, GAD-7) to guide treatment decisions - Document clinical outcomes and adjust care plans accordingly - Coordinate care with internal and external providers as ne...
