Job description

Overview In collaboration with departmental and organizational leadership, supports the organization’scommunity reinvestment activities through active and meaningful engagement in identifiedcommunity workgroup and initiatives. On behalf of the health plan identifies and supports, keystrategic activities and interventions that support alignment of collective agency efforts thatpromote and support efforts to encourage member disparity health outcomes. Responsibilities: ▪ Works collaboratively with leaders and community partners in the development of thehealth plan’s community reinvestment plan through active participation in the LocalHealth Department’s Community Health Assessment (CHA) and/or Community HealthImprovement Plan (CHIP) processes and Behavioral Health Transformation plans▪ Helps to develop, track, and monitor the health plan’s shared reinvestment plans foreach assigned county▪ Conducts outreach and engagement to build relationships with stakeholders including,but not limited to: Local Health Departments, hospitals, providers, community-basedorganizations (CBOs), education agencies, grass roots organizations, and other externalpartner agencies.▪ Fosters positive working r...
