As the "Air Traffic Controller" of our office, you ensure that patients transitioning from hospitals to their homes receive care without delay. This is a fast-paced role that requires someone who can juggle logistics while maintaining a compassionate touch. The Schedule: Consistency + Flexibility: To provide world-class care, our office remains responsive seven days a week. This is a dedicated, full-time position.
