Sun River Health is currently seeking a full-time Social Care Network Care Manager to join our Health Home team in Suffolk County. Social Care Network Care Manager (SCN Care Manager) – Temporary Role Details: The SCN Care Manager supports the implementation of a patient’s integrated health care plan and works toward resolving patient needs and barriers. This role conducts screening and navigation services for patients eligible for the Social Care Network, with documentation completed in Unite Us and eCW. The focus is on providing individualized assistance to help patients navigate the health care system and community barriers, facilitating consistent and timely care across the continuum.
