Job description
The PT will be able to identify patients/customers: Inpatients, Outpatients, Home Health/Hospice patients, Sub-Acute Unit patients (neonates to geriatrics). The PT is responsible for performing an age-specific assessment designed to gather data relevant to the patient's individual needs. This includes assessment and evaluation of cognitive, sensory, neurological, musculoskeletal, functional, and physiological status. The PT develops a plan of care, performs treatment as indicated, assesses patients' response to treatment, and assists with discharge planning. The PT utilizes exercise, equipment, and physical agents to improve strength, balance, coordination and range of motion, and to decrease pain to restore a patient's optimal mobility and functional ability. The PT provides education related to the treatment program, disease process, and prevention of further injuries to the patient, family/caregiver and other members of the healthcare team. Depending on the location, the PT will be expected to delegate and monitor treatment provided by Physical Therapist Assistants.
