Reliance Hospice and Palliative Care is currently hiring for a FULL-TIME RN Case Manager to support our patients around Corona area (Riverside, South Corona, Colton, Moreno, 60 freeway, and surrounding cities ) Hourly Salary starts at $51 and is negotiable depending on years of experience as RN. Hospice experience is required. The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Job Responsibilities: - Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. - Assesses and evaluates patients status. - Follow plan of care and participate in revising the plan of care as necessary. - Regularly re-evaluating patient and family/caregiver needs - Provides education, instructions and support to the patient and family on medication and patient care related needs. - Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to...
