The Team: The Hospital@home South service plays a vital role in the communities of Southern Tasmania. It offers hospital-avoidance interventions and both acute and subacute care for adults, ensuring safe, effective, multidisciplinary, hospital-level care within the community. The service operates across multiple sites throughout Southern Tasmania. Hospital@home: South satellite sites are currently located in Cambridge, Glenorchy, and Sorell. The Role: The Registered Nurse - Community: Consistent with ongoing experience, strengthens health outcomes through the provision of safe, quality, clinically appropriate nursing care in partnership with patients/clients, their families and other health professionals within the defined community practice area.
