Description Conifer Park is looking for novel clinicians who are ready to change lives in our licensed facility located in Troy, NY. We provide outpatient treatment, with cutting edge support for those struggling with substance-use-disorder (SUD). A Conifer Park clinician provides comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; discharge planning; and intervention with client support systems, including social service programs and managed care. 5,000k Sign-on bonus eligible RESPONSIBILITIES : Schedule : Full Time - 40 Hours per Week. Range : 7:00am- 7:00pm - Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures. - Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. - Documents patient progress and own interventions in case record.
