Job description

The Clinical Pharmacist Specialist - Critical Care is responsible for demonstrating proficiency in all pharmacist functions, such as interpreting, transcribing, or verifying medication orders. Attend and actively participate in daily multidisciplinary and bedside rounds, complete drug utilization reviews to promote rational drug therapy, lead and develop clinical services, and mentor and educate staff to ensure accurate and appropriate documentation of clinical activities as needed. EDUCATION: CERTIFICATION/LICENSURE: - Responsible for participating and leading pharmacy clinical/operational programs, monitoring patient medication regimens and providing appropriate interventions. - Review, interpret, and input treatment orders to ensure accuracy of dosage, treatment schedule, route of administration, and safe administration based on laboratory information and evidence based practices. - Provides and supports clinical pharmacy responsibilities by collaborating with other hospital disciplines to support service lines - Lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosin...
