As a home health COTA, you’ll join a multi-disciplinary home health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive medical care at home. How You’ll Make an Impact: Be partnered with an experienced and supportive home health Occupational Therapist Follow established care plans and work with patients 1:1 to improve function, ADL, and IADLs Educate and instruct patients, family members, and other patient representatives in rehabilitative care and activities necessary to support the client’s health, safety, and independent living Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-o...
