The Role We currently have an exciting opportunity for a highly motivated, person orientated individual to join our organisation as a Social Worker. This is full-time, spanning across two of our facilities: Lefroy Care Centre (BullCreek) & Mosman Park Care Centre(Mosman Park), Monday - Friday. In this dynamic role, you will play a crucial role in promoting the psychological safety and well-being of clients / residents and their representatives as the access Amana Living Services. This position provides vital information and support tailored to each individual’s complex care and social circumstances. The Social Worker assists residents and their families in understanding their care needs and navigating the intricacies of care planning, decision-making and provision of community community-based services. Additionally, you will collaborate with a multi-disciplinary team to empower clients and their families to achieve their goals affectively.
