This is a part time position working 20 hours per week. The Social Worker will provide professional social work intake/referral, and discharge planning services for Lakeview Personal Care while working as a member of Personal Care's resident caregiving team to provide psychosocial evaluations and facilitate appropriate team responses to residents' needs. This individual will coordinate services to enhance residents' length of stay and placement when residents' needs are no longer appropriate to the Personal Care level of service.
