This is NOT a remote position Title: Weekend Home Health RN (Field) Schedule: Per Diem (12 RVE p/week) must work Saturdays and Sundays 9am-5:30pm Locations: Area #4 covers the following cities: Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek AND Area #5 covers the areas of Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, Bay Point AND Area #3 covers the areas of San Ramon, Dublin, Pleasanton, Sunol, Livermore Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm in San Ramon (onsite) Position Summary: The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients.
