Description JOB SUMMARY: Within the context of a patient/client and family-centered framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Occupational Therapist plans, delivers, evaluates and provides care coordination to address the needs of the clients and families experiencing functional impairment and acute/chronic disease within community, ambulatory and/or institutional settings.Collaborates with family members or significant others, interdisciplinary team members, educational resources and other healthcare professionals and/or providers to address the client’s functional/physical/occupational needs and social/cognitive/perceptual/environmental needs, to maximize capabilities, community participation and recovery from or adaptation to illness/impairment to achieve excellence in health services and education.
