Community Health Representative (CHR/MVO)

Job description

Job Purpose & Position Overview: The purpose of this position is to provide contact, referral and transportation for general medical patients and residents of the Winnebago community to also include patients being transported to the Dialysis centers. The CHR/MVO may be required to assist in developing routine programs aimed at benefitting the greatest number of community members as well as provide weekly projections for work schedule and prepare/maintain progress reports on all program involvement of clientele. Employee shall work on an eight (8) hour shift, which may require decreased or no less than 40 hours per week and be responsible for after-hours on-call duties on a rotating monthly basis for CHR, MVO and EMS.
