Department: 39122 Central Metro E - Home Care Physical Therapy Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Monday through Friday; Weekend rotation, 1 Summer , 1 Winter holiday requirement Pay Range: $44.15 - $66.25 Physical Therapist (PT) – Home Health Case Manager Location: Chicago North Side (Irving Park, Albany Park, Uptown, Lakeview, and surrounding areas) Schedule: Day shift with weekend and holiday rotation Sign-On Bonus: This position is eligible for up to a $10,000 sign-on bonus . External candidates only. Must be employed in the same role at the time of payout. This position is eligible for up to a $10,000.00 sign on bonus.
