Provides support and resources to Family Home Providers caring for individuals with developmental disabilities in their homes. Essential Functions and Responsibilities: Completes home visits to ensure quality of care, client satisfaction, and compliance with regulations. Facilitates ISP meetings and discussions about clients' goals with clients and members of the support team. Prepares written plans detailing client goals and areas where support is needed, and updates as needed. Provides training and guidance to families regarding appropriate support to meet client goals and care needs. Maintains charts according to specifications in home and office locations. Communicates pertinent information to all members of the support team. Provides on-call support to families outside of regular business hours, as needed.
