Description The Children’s Behavioral Health Home (BHH) Coordinator / Targeted Case Manager is responsible for overseeing and coordinating comprehensive care services for children and families with behavioral health needs. This full-time role blends direct client support with care coordination, documentation, advocacy, and collaboration with multidisciplinary teams to ensure high-quality, person-centered care. The ideal candidate is compassionate, organized, and skilled at engaging children and families — including those who may be hard to reach or hard to engage — while navigating complex systems of care.
