Position Overview: As a Hospice RN Case Manager at Mount Carmel Hospice, you’ll deliver one-on-one, high-quality care to patients in the comfort of their homes. Using advanced technology and your clinical expertise, you’ll assess, plan, and manage individualized care that promotes healing and independence. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.
