Chestnut is seeking nurses to drive integrated care across primary care and psychiatric programs. This candidate will be based in our Granite City Health Center but may occasionally support Maryville, or Belleville. The schedule for this role is Monday- Friday, typically 8 am to 5 pm. The anticipated starting pay for new hires for this position is between $19.85 - $40.84 per hour. Responsibilities: Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness. - Carry out health care plans designed by practitioners. - Administer treatments, medications, and medical tests. Gather patient health histories. - Assess patients for acute physical issues. - Act as a liaison among prescribers, patients, laboratories, PCPs, other providers, pharmacies, and staff to ensure continuity of care. - Provide health teaching as well as support and guidanc...
