Location: Sacramento,California,United States The Care Coordinato r, Enhanced Case Management services is responsible for developing and providing high quality therapeutic interventions and case management services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. This position is based at our Camellia Village 42 unit independent SIL housing program in Sacramento. This is a community-based position providing direct, face-to-face services to individuals who are chronically homeless and travel throughout Sacramento County is required. Schedules include: - Shifts are Sunday to Thursday and Tuesday to Saturday.
