System Navigator 1 Casual Thunder Bay Site One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking one (1) casual System Navigator - Gender Affirming Care Group Facilitator up to 5 hours per week. Gender Affirming Care is embedded in the work we do as a Community Health Centre and the facilitator is a valuable member of the Community Health Centre team will provide supports for clients to improve health and well-being, reduce social isolation through the delivery of programs and connect clients with long-term services in the community. The Community Health Worker works closely with the inter-professional care team and community partners to support clients with their needs and goals related to the determinants of health. The position has a competitive salary, the Health Care of Ontario’s Pension Plan and an excellent group health benefits package. The hourly range for this position is from $30.502 to $36.748.
