Location: Geisinger CenterPoint Office Building Shift: Days (United States of America) Scheduled Weekly Hours: 40 Worker Type: Regular Exemption Status: No Job Summary: Works within the community providing outreach to patients with complex medical and social needs. Acts as a resource to assist patients in navigating the health care system. Job Duties: Work is typically performed in a home environment.
