Our Reablement Service is looking for a passionate Team Manager to join us in the continued development and management of reablement provision. The Reablement Service works in an integrated relationship with our health partners. We provide a discharge to assess pathway that supports people to return home from hospital in a seamless whole system approach called Wiltshire Reablement and Homefirst Services. The Reablement team also provides and social care reablement and equipment to the residents of Wiltshire. As a Reablement Team Manager, you will line manage the staff within Operational aspect of the service, including Occupational Therapists, Occupational Therapy Assistants, Physiotherapists and Social Care practitioners in the team.
