Salary The salary range for this position is CAD $44.85/Hr. - CAD $56.02/Hr. Job Accuracy Posting Flag: The hours of work, including start and stop times, days off, work area and work site may be subject to change consistent with operations requirements and applicable provisions of the Collective Agreement and statutes. **Testing may be required. ***A criminal record check may be required Summary: Reporting to the designated Social Work Site Leader and receiving guidance and support from the Professional Practice Leader and/or Research, Education and Practice Coordinator, the position supports the interdisciplinary program/team, which is under the direction of the Patient Care Manager, in achieving program, patient and resident goals. - Providing therapeutic clinical counselling services, conducting comprehensive biopsychosocial assessments and maintaining patient/resident records. - Provides consultative and direct services to patient/resident, families, staff and community health professionals regarding community resources and provides leadership to the interdisciplinary team in the discharge planning process. - Coordinates clinical/case conferences, develops and delivers in serv...
